Technology and Teamwork Key to Faster Hospice Admissions

Rising demand and severe workforce shortages have made it harder for dying patients across the country to get timely access to hospice care. But some hospices are finding ways to speed up the process by leaning on technology and better collaboration between teams.

When a patient is referred for hospice, every hour matters. Delays in admission can mean unnecessary suffering for patients and added stress for families. Yet many hospices still rely on manual paperwork, phone tag between doctors and nurses, and outdated scheduling systems. These bottlenecks can stretch a process that should take hours into days.

Hospices that have adopted digital tools are seeing real improvements. Electronic health records integrated with referral platforms allow intake teams to verify eligibility, review medical histories, and coordinate with attending physicians without endless phone calls. Mobile apps let field nurses receive assignments instantly and document visits in real time, cutting down on back-office delays.

But technology alone is not enough. The most successful hospices also restructure how their teams work. Instead of having separate departments for referrals, clinical assessments, and scheduling, they create cross-functional teams that handle each admission from start to finish. This reduces handoffs and miscommunication. Some organizations now have dedicated "rapid response" nurses who can be at a patient's bedside within hours of a referral.

The result is faster admissions, fewer dropped referrals, and better care for patients who have little time to wait. As demand continues to grow and staffing remains tight, these combined approaches are becoming essential rather than optional.
